Integrated Behavioral Health Services
Our Integrated Behavioral Health Services range from improved coordination with medical providers to innovative programming in medical settings. We have Social Work Care Coordinators/Behavioral Health Social Workers as part of the regional Blueprint for Health Community Health Team embedded in both primary care practices and specialty care practices. We also have a partnership with our region’s Federally Qualified Health Center (FQHC), the NOTCH sites. We continue to develop our partnership with Northwestern Medical Center (NMC) through a variety of services and planning efforts.
Integrated Behavioral Health Servcies include, but are not limited to:
- Short-term therapy directly in the primary or specialty care practice
- Health Assessment and screenings
- Developing strategies to manage conditions which include goal setting to improve personal health; symptom management of anxiety and depression, etc; Nutrition support; Tobacco Cessation support; Identification of mental health barriers to meeting other health care goals; Introduction and practice of self-care strategies
- Understanding benefits
- Coordination of care with other community partners and navigation of services
- Guidance and support for healthier living
- Specialty referrals and navigation of other services
