Traumatic Brain Injury

The Developmental Services Division provides supports through the TBI Program.  The goal of this program was to provide Vermonters with a moderate to severe traumatic brain injury, supports to prevent utilization of out of state facilities.    The TBI program at NCSS provides supports that are tailored to the individual’s specific needs.   These can include:   Case Management, Rehabilitation Services through the support of Life Skills Aides, the support with coordinating and medical follow up, community support, crisis support, respite support,  home support, employment and counseling

Eligibility requirements include:

  1. Receive Traditional and/or Long Term Vermont Medicaid
  2. Must be a resident of Vermont
  3. Age 16 or older
  4. Diagnosed with a moderate to severe brain injury result in in residual deficits and disability.   Injury must have occurred within the past five years.  

To apply for this program a completed application has to be submitted to the TBI Program Manager at the Adult Services Division in Waterbury. For more detail on eligibility requirements you can visit asd.vermont.gov
