Vermont Care Partners (VCP) is a statewide network focused on supporting individuals with intellectual and developmental disabilities (I/DD), as well as those with mental health and substance use needs, to maintain independence and well-being. While not traditional housing providers, VCP agencies help individuals navigate complex systems and find appropriate housing solutions. Northwestern Counseling & Support Services (NCSS) is expanding its housing options to serve individuals with I/DD, particularly those with higher medical and mental health needs. In early 2025, NCSS will open the Able House, a six-bed residential care home in Franklin County, designed for individuals aged 45 and older with I/DD who require more medical support than typical providers can offer. This facility will provide 24-hour person-centered care, including nursing oversight, medication management, and psychiatric services, allowing individuals to live in their communities while receiving the specialized care they need. The Able House addresses a critical need for supportive housing as many aging individuals with I/DD face increasing complexity in their medical and personal needs, often outgrowing the traditional Shared Living Provider model. This project offers a vital resource to prevent housing insecurity, reduce the need for out-of-town placements, and ensure individuals can age in place with the care and support they deserve.
